Job summary

Sirona care & health CIC is seeking a Nurse Associate or Assistant Practitioner to join our community teams in Bristol, North Somerset or South Gloucestershire. You will work across a rota system 8am–8pm, supporting multi-disciplinary teams to assess and plan care for acutely and chronically unwell patients.

You will need excellent communication and clinical skills, with experience in electronic records, prioritising workloads and sharing knowledge with learners.

Qualifications

• Higher Apprenticeship in Health level 5 or Foundation Degree in Health and Social Care.
• Current NMC registration (if Nursing Associate).
• Willing to undertake relevant training for the role.

Responsibilities

• Undertake initial holistic assessments and implement care programmes.
• Assess and monitor service users' health needs and wellbeing.
• Use clinical reasoning to plan and evaluate patient-centred care.
• Collaborate with service users, carers and family to set goals.
• Facilitate referrals to other services as needed.
• Work within a multi-disciplinary team to provide integrated care.
• Assess and arrange equipment provision.
• Educate service users and carers to promote independence and safety.
• Maintain safe, clean environments and accurate records.
