Job summary

Derbyshire Community Health Services NHS Foundation Trust seeks Health Visitors to deliver the Healthy Child Programme across the North High Peak area. The role requires SCPHN Health Visiting and V100/V150 nurse prescribing qualifications, with travel across a rural locality and occasional meetings in Derbyshire.

You will manage caseloads, address safeguarding, assess needs, and develop care programmes while supporting the local 0-19 team that has Gold accreditation.

Qualifications

• Experience in delivering the Healthy Child Programme.
• Ability to manage safeguarding within caseloads.
• Capacity to plan, implement and evaluate care programmes.

Responsibilities

• Deliver the Healthy Child Programme to families and communities.
• Manage a defined caseload and address safeguarding issues.
• Assess needs and develop, implement and evaluate care plans.
• Prescribe within the nurse prescribing formulary as appropriate.
• Lead and develop Health Visiting support staff.
