NHS Greater Glasgow and Clyde (NHSGGC) is seeking a compassionate Community Health Care Assistant to join the Adult Community Nursing Service based at Eastwood Health and Care Centre. A full UK driving licence and access to a vehicle are essential for this permanent, full-time role (36 hours per week).

Qualifications

• SVQ Level 3 in Healthcare or equivalent healthcare experience.
• Experience of working directly with patients and providing person-centred care.
• Ability to work independently and as part of a multidisciplinary team.
• Strong verbal and written communication; ability to build positive relationships.
• Knowledge of patient confidentiality, record keeping and professional standards.
• Ability to prioritise workload and recognise when to seek support.
• Full UK driving licence and willingness to travel across community locations.

Responsabilités

• Deliver delegated clinical interventions for patients at home and in community clinics.
• Undertake duties such as phlebotomy, blood pressure monitoring, oxygen saturation recording and specimen collection.
• Monitor patient wellbeing, recognise changes and escalate concerns appropriately.
• Maintain accurate patient records and ensure confidentiality at all times.
• Work collaboratively with GPs, community nursing teams and wider health and social care professionals.
• Support patients, families and carers to promote independence and wellbeing.
• Participate in training, service development activities, audits and team meetings to support continuous improvement.
