Job summary

Oxford Health NHS Foundation Trust is seeking a District Nurse Triage professional to join our community care team. You will triage referrals by phone and email, determine urgency, and coordinate appropriate district nurse visits in the community setting.

Qualifications

• Registered General Nurse (adult) with current UK registration.
• Educated to degree level or equivalent.
• IT literate with basic keyboard skills and ability to use clinical governance systems.

Responsibilities

• Triage referrals received by telephone and email.
• Assess urgency and plan care levels using clinical judgement.
• Coordinate urgent, same-day and routine district nurse visits.
• Provide clinical advice when a face-to-face visit is not needed.
• Work with District Nurse teams and community services to ensure safe care coordination.
• Contribute to ongoing development of the District Nurse Triage Desk and future community care delivery.
