Job Overview

In this specialised role, you will be working as a Community Diabetes Specialist Nurse within Bath and North East Somerset area. You will contribute to the wider strategic development in diabetes care with colleagues, working in partnership with the Royal United Hospital and the Bath Swindon and Wiltshire (BSW) Sustainability and Transformation Partnership (STP). There will be an opportunity to develop your skills with supporting persons on insulin pump therapy and diabetes technologies.

Job Overview

In this specialised role, you will be working as a Community Diabetes Specialist Nurse within Bath and North East Somerset area. You will contribute to the wider strategic development in diabetes care with colleagues, working in partnership with the Royal United Hospital and the Bath Swindon and Wiltshire (BSW) Sustainability and Transformation Partnership (STP). There will be an opportunity to develop your skills with supporting persons on insulin pump therapy and diabetes technologies.

Due to the nature of this role, we require candidates who apply to have current right to work in the UK.

Main duties of the job

You will provide valuable, expert resources for our patients with diabetes, medical, nursing and allied healthcare professional staff, facilitating communication between primary, secondary care, the integrated team and STPs. You will manage a caseload of patients providing advanced clinical support, advice, treatment and education for people, avoiding hospital admission, by prompt intervention and treatment.

Qualifications & Training

Essential criteria

  • Registered Nurse – level 1
  • Accredited diabetes qualification
  • Independent nurse prescribing or willing to undertake
  • Evidence of training at degree level and ongoing continuous professional development

Desirable criteria

  • Communication skills course
  • Participate in conferences, training associated with diabetes

Knowledge & Experience

Essential criteria

  • At least 2 years’ experience working with persons with diabetes either within the community or acute setting
  • Up to date knowledge of nursing theory and best practice, particularly within diabetes nursing and an understanding of its application in clinical practice
  • Understanding of NMC Code of Practice and requirements of it for the practice & behaviour and its application to the management of staff and self
  • Understanding of equality & diversity and its application to managing self, staff and patients within own area
  • Computer skills including downloading diabetes devices and interpreting data

Desirable criteria

  • Experience managing and supporting persons diabetes
  • Membership of special interest and professional group

Requirements due to Working Environment

Essential criteria

  • Knowledge of policy and procedure dealing with blood/urine and safe disposal of sharps

Desirable criteria

  • Flexible working hours

Specific Skills

Essential criteria

  • Ability to work autonomously and as a team members and promote collaborative multi-disciplinary ad multi agency working
  • Ability to manage time effectively; meet deadlines and deal with work pressures and demands
  • Knowledge of evidence based practice for the management of diabetes
  • Typing skills - to use computerised system for accessing patient information
  • Counsel and support patients with ongoing physical and psychosocial problems related to Diabetes

You must have appropriate UK professional registration.

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
