Nottingham CityCare Partnership's Housebound Continence team is seeking a registered nurse to provide community continence assessment and treatment for housebound adults. You will work in patients' homes with supervision from band 6/7 nurses while line managing an assistant practitioner.

The role involves clinical assessment, patient advocacy, and travel across the city to visits and training, within daytime hours and with a driving licence and transport required.

• Complete holistic continence assessments and re-assessments for adult patients in community settings.
• Translate assessment outcomes into individualised management and treatment plans, including onward referrals.
• Implement care and evaluate effectiveness.
• Provide continence advice through clinical practice, teaching and research.
• Hold a defined clinical caseload under supervision of specialist nurses.
• Maintain contemporaneous records as an autonomous practitioner.
• Line manage and support assistant practitioners; supervise student nurses on placement.
• Gain valid informed consent and work within the legal framework.
• Travel city-wide for patient visits, meetings and training.
• Organise own caseload, diary and priorities.
• Engage in audits, service evaluation and quality improvement.
