Job summary

Discovery is seeking a Community Connector Outreach Support to empower adults with learning disabilities or autism across Street and Yeovil. You will provide independent, person-centred support, help people access activities, relationships, volunteering and employment, and promote community involvement.

The role is lone-working with flexible shifts from 8:30am to 10:00pm, requiring a full UK driving licence and access to a vehicle for work. Training and DBS checks are provided.

Qualifications

• Full UK driving licence required.
• Access to own vehicle and willingness to use for work.

Responsibilities

• Provide face-to-face support to 24 adults with learning disabilities or autism.
• Assist with day-to-day travel arrangements and promote independence.
• Support personal care and occasional medication support as needed.
• Maintain accurate records using IT systems; build relationships with families and community networks.
