Community Clinical Support Worker - Adult Community Mental Health Team - East Suffolk

A great opportunity has arisen to join Coastal Community Mental Health Adult Team based in Ipswich covering the East of Ipswich through to Felixstowe, Aldeburgh and Leiston areas as Community Clinical Support Worker. We are a recovery focussed, multi-disciplinary team providing an effective, supportive, and professional service for our Services users and Carers.

The team is community based, working with individuals in their homes and the wider community working to provide the best care for service users under our care. You would be supervised and supported by registered professionals in the team and be working both independently and supervised with service users . The successful applicant would be required to travel independently between service user homes and communities. A full UK driving licence would therefore be required.to fulfil these duties.

Main duties of the job

Undertake a range of delegated clinical health duties in relation to patient care in the community under the direct supervision of a Registered Practitioner or Assistant Practitioner.

Undertake basic home visits and report back to Registered Practitioners after face to face contact with service users.

Care for the patient's physical, psychological, social, spiritual, cultural and recreational needs within a defined programme of healthcare ensuring that the patient's dignity is maintained at all times.

Observe, report and record any changes, which may indicate an alteration in the patient's wellbeing as well as any routine observations required.

Undertakes medicine drops to service users.

Engage in joint working with Registered Practitioner e.g. support practitioner on assessments where a lone visit would not be appropriate.

Support service users in participating in daily activities within the community e.g. banking, shopping, community groups

Undertake home visits approved and risk assessed by the Registered Practitioner subject to regular review through regular supervision.

Complete the Care Certificate within 12 weeks of employment and commence a L2/3 H&S care framework if this has not already been achieved as this qualification is required for this role
