Job summary

Norfolk Community Health and Care NHS Trust is seeking Clinical Support Workers for the newly opened Willow Unit at Norwich Community Hospital. In this role, you will support patient reablement and recovery within a multidisciplinary team, providing high-quality personalized care to ensure patients achieve their independence.

Qualifications

NCH&C is hiring Clinical Support Workers within the state-of-the-art Willow Unit. Recently opened at Norwich Community Hospital, the new facility provides a supportive and comfortable environment for 48 patients.

The restorative environment at Willow Unit enables patients to make steady, step-by-step improvements and gain independence as they prepare to return to the community. Patients are supported to play an active role in their own care and feel confident about performing daily activities when they leave.

Healthcare is delivered by a multidisciplinary team of clinical professionals dedicated to patient recovery and wellbeing.

Main duties of the job

Utilising the latest therapy and rehabilitation technology and practices, you will be a part of this new team and will play a pivotal role in supporting patients back into the community and, in this role, we can offer you the opportunity to play a significant part in creating a reablement culture within a brand-new unit.

Work as part of the team to deliver care in a reablement setting.

To support an environment where all team members are responsible for a reablement ethos using a positive risk-taking approach for patients, their carers and families.

To support patients being as independent as possible with their washing/dressing and nutritional needs.

To work as part of a multidisciplinary team within a quality framework to ensure that the highest standards of care are met.

To support and deliver innovation within discharge practice and pathways.

To work with system partners including the voluntary sector to ensure that patients experience a seamless transfer of care both into and onward from the unit.

Support the delivery and co-ordination of care to a named group of patients.

Work proactively within the team to achieve unit and trust goals and objectives.

Work with and promote recognised policies, protocols, and guidelines.
