Qualifications

• Must be a registered nurse or allied health professional.
• Autonomous working with a multi-disciplinary team.
• Experience working with children and young people is desirable.

Responsibilities

• To work autonomously.
• Provide professional leadership and clinical guidance within health care quality dimensions (patient safety, effectiveness, patient experience, environment).
• Assess, plan, implement and evaluate programmes of evidenced-based care for a group of service users with varying complexity.
• Work within a multi-disciplinary team to ensure effective care plans and treatments for complex service users.
• Recognise, prioritise and respond to urgent and emergency situations.
• Collaborate with other professionals to meet care needs and support safe discharge.
• Maintain accurate, complete and contemporaneous clinical records.
• Contribute to development, implementation and delivery of new ways of working and research-based practice.
• Provide a personalised service while promoting quality, diversity and rights.
• Consider the impact of the care environment on patient well-being and care delivery.
• Communicate complex and sensitive information with tact where needed.
