Job summary

Leicestershire Partnership NHS Trust (LPT) is recruiting a highly motivated clinical practitioner (Registered Nurse or AHP) to oversee a cohort of service users with complex needs within the Dynamic Support Pathway.

You will work autonomously within a multi-disciplinary team, liaise with other agencies, and maintain accurate clinical records while helping develop care plans and new care models to improve outcomes for adults and children with learning disabilities and/or autism.

Qualifications

• Must hold a professional qualification and be able to work autonomously in a multi-disciplinary team.
• Excellent communication and interpersonal skills are required.
• Experience of working with children and young people is desirable.

Responsibilities

• Provide professional leadership and clinical guidance within the team.
• Coordinate care plans, undertake assessments and evaluate programmes of care.
• Collaborate with other professionals, agencies and stakeholders to ensure personalised care.
• Maintain accurate, contemporaneous clinical records in line with policy.
