Job summary

Leicestershire Partnership NHS Trust in Leicestershire is recruiting a clinical practitioner (Registered Nurse or AHP) to join a small team. The role focuses on preventing unnecessary admissions to mental health hospitals for people with learning disabilities and/or autism, coordinating care for a complex caseload.

You will work autonomously within a multi-disciplinary team, develop effective care plans, communicate clearly with colleagues and agencies, and keep detailed clinical records.

Qualifications

• Qualified professional (RN or AHP) with registration and ongoing practice requirements.
• Outstanding communication and interpersonal skills essential for multi-disciplinary team work.
• Ability to problem solve, prioritise and uphold trust values in all activities.

Responsibilities

• Provide professional leadership and clinical advisory support within the team.
• Plan, implement and evaluate care for individuals with complex needs.
• Coordinate care across agencies and ensure effective discharge planning.
• Maintain accurate, complete, contemporaneous clinical records.
• Promote evidence-based practice and contribute to service improvements.
• Deliver personalised patient-focused care in line with agreed standards.
