Job summary

NHS Humber and North Yorkshire Integrated Care Board (ICB) is seeking a qualified RN/RMN/RNLD to join its Continuing Healthcare team in a non-clinical/clinical change role. The post focuses on end-to-end CHC pathway delivery, assessments, case management, and MDT collaboration within the Hull & East Riding area.

Responsibilities

• Provide comprehensive clinical advice, assessment and case management for CHC and FNC.
• Follow NHS National Framework for CHC & FNC for assessments and reviews.
• Procure safe and appropriate care packages based on individual needs.
• Maintain accurate documentation and data flow for payments and governance.
