Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking a Community Nurse to provide clinical and operational leadership within the community nursing team. The post holder will support the Integrated Senior Clinical Lead to deliver safe, patient-centred care and coordinate management for patients with complex needs.

Responsabilités

• Provide clinical and operational leadership within the community nursing team.
• Manage and coordinate a caseload of patients with complex health needs.
• Undertake holistic assessments, care planning, implementation, and evaluation of care.
• Support patients to remain independent and avoid unnecessary hospital admissions.
• Work closely with GPs, Social Care, therapists, and other healthcare professionals to ensure coordinated care.
• Provide clinical advice, supervision, and support to junior staff and students.
• Monitor patient safety, risk management, safeguarding, and clinical quality standards.
• Deputise for the Band 7 Integrated Senior Clinical Lead when required.
