Job Description:
  • Work with clients and their families/carers to identify and mobilise their strengths, networks, communities, and help maximise independence, self-care, social, and occupational opportunities as appropriate.
  • Collaborate with other professionals including social workers and occupational therapists, referring clients as necessary.
  • Assess and review individual needs, arranging the provision or referral of services, including prescribing and delivering simple equipment and non-complex housing adaptations in line with preventative approaches.
  • Review work and caseloads regularly with the Service Manager, including case closures and transfers.
  • Conduct holistic assessments for individuals with less complex needs to identify unmet outcomes and help create care and support plans in accordance with legislation.
  • Provide information to clients about available resources, linking them to local community resources and activities as appropriate.
  • Liaise with voluntary bodies, residential and day establishments, and other agencies.
  • Ensure safeguarding of vulnerable adults.
  • Maintain records according to DCC policy.
  • Commission services internally and externally based on assessed needs.
