Family Outreach Worker (Edge Of Care) (Social and Healthcare Qualified), Huddersfield

This role involves working directly with children and families in their homes and communities, identifying appropriate interventions to deliver positive outcomes. The work includes early mornings, evenings, and weekends as needed.

The responsibilities include conducting holistic assessments, devising and implementing plans for children and families, and supporting families with complex needs such as substance abuse, mental health issues, and domestic abuse. The worker will review support packages regularly, maintain accurate records in line with GDPR, and participate in meetings, reports, and reviews.

The role also involves providing timely advice, supporting participation of children and young people, engaging in professional development, and adhering to policies and procedures. Additionally, the worker may support students, apprenticeships, and volunteers, and will be responsible for petty cash and equipment access as required.
