Job Title: CNA / Certified Nursing Assistant - Hospice

Location: US-PA-READING

Overview

Our Hospice Aide/CNAs are vital to our organization, supporting patients with personal care services to meet their needs. Responsibilities

1. Perform personal care and attend to patient requests promptly, reporting to the Case Manager if unable to perform a task.
2. Communicate positively with patients and families, maintaining confidentiality and ethical standards.
3. Ensure safety and proper use of equipment such as foot stools, side rails, and oxygen.
4. Provide personal care including baths, oral hygiene, shampoos, and changing linens.
5. Assist with dressing, feeding, and mobility as instructed.
6. Record vital signs and observe patient skin condition.
7. Report changes in patient condition to the Case Manager.
8. Assist with medication reminders, light laundry, and simple procedures as ordered.
9. Maintain a clean and orderly environment and adhere to documentation standards.
10. Participate in quality improvement activities and respect patient and family needs.

Qualifications

1. Meet state training requirements with a minimum of 75 hours of classroom and supervised practical training.
2. At least six months of experience providing personal care.
3. Be at least 18 years old, with the ability to read, follow instructions, and document care.
4. Understand hospice philosophy and comfort with terminally ill care.
5. Possess current CPR certification, demonstrate tact, patience, good hygiene, and good communication skills.
6. Successfully complete a probationary period demonstrating competence in job qualifications.
