Schedule: PRN

How You Will Contribute

• Coordinate with the assigned nurse to provide safe patient care and report on the patient’s condition.
• Perform personal care, take vitals, communicate with families, and other duties as assigned.
• Travel between patients and work as part of an interdisciplinary team supporting families and patients during their end-of-life journey.

Qualifications

• Meet state training requirements
• Minimum six months experience providing personal care
• At least 18 years old
• Complete 75 hours of classroom and supervised practical training (16 hours prior to practical training)
• Current CPR certification
• Ability to read and follow written instructions and document care
• Understanding of hospice philosophy and comfort with caring for the terminally ill
• Ability to work with minimal supervision
• Ability to handle death/dying
• Strong communication and organizational skills
