Job summary

Thriving is recruiting for a Registered Nurse – Clinical Team Leader to provide day-to-day clinical leadership across our community-based care services in Manchester. You will oversee medicines governance, lead training for Support Workers and ensure safe, person-centred care in homes.

Qualifications

• Active NMC PIN required.
• Experience in complex or domiciliary care desirable.
• Strong risk assessment and care planning skills.
• Experience training or supporting care staff.

Responsibilities

• Provide clinical leadership across community-based care packages.
• Complete clinical assessments for new service users and set up new packages.
• Write and review person-centred support plans and risk assessments.
• Lead training, coaching and development of Support Workers.
• Assess and sign off clinical competencies and delegated tasks.
• Provide package-specific supervision and support to Support Workers.
• Lead medicines governance and conduct audits.
