Clinical Team Leader - Community Mental Health

The closing date is 16 June 2025

An opportunity has arisen to join Queens Park and Paddington Community Mental Health Hub (CMHH) as a Clinical Team Lead.

We are looking for an experienced Mental Health Nurse/OT to join the team to work in the community with Adults with mental health needs and support the team with the day to day running of a busy mental health service.

The role is suitable for nurses/OT's who have experience in working in the community and for someone seeking to develop their leadership and management skills.

Please note that this role requires successful candidates to be able to engage in Therapeutic Management of Violence and Aggression (TMVA) training, including the use of force techniques and the ability to attend resuscitation training and perform CPR. This is because of the high-acuity nature of the ward/community environment and the need to ensure the safety of both patients and staff. The role is conditional upon candidates being able to engage in TMVA training and resus training during the course of their employment whilst undertaking this role.

Main duties of the job

As Clinical Team Lead you will be accountable for the day-to-day management of the Hub, overseeing referral, assessment, allocation and discharge pathways to ensure timely, intervention-led, evidence-based and person-centred care.

You will hold a small clinical caseload, delivering and supporting multidisciplinary interventions, chairing MDT meetings and leading on risk assessment, safeguarding and crisis planning in line with Trust policy.

Working closely with the Hub Manager, you will drive service governance through audit, quality-improvement initiatives and performance-data audit, and ensure compliance with CQC standards.

You will also support the management of operational functions such as incident investigations, complaints and escalations. You will provide supervision, appraisals and professional development for the team, and oversee recruitment, induction and training.

You will collaborate with inpatient wards, Home Treatment Teams, primary care, social services and voluntary sector partners to ensure seamless care pathways and drive ongoing service improvement.

The successful applicant may have contact with patients or service users. As an NHS Trust we strongly encourage and support vaccination as this remains the best way to protect yourself, your family, your colleagues and of course patients and service users when working on our healthcare settings.

About us

You will receive monthly clinical supervision, an annual appraisal with a personal development plan and access to a broad suite of internal and external training opportunities.

We provide structured career pathways such as preceptorship and revalidation support and celebrate achievement through Hidden Gem and annual awards ceremonies. Our generous benefits package, among the best in London, includes health and wellbeing services, season ticket loans and a cycle to work scheme.

CNWL is recognised nationally and internationally for high quality, innovative care; our experts contribute to national health strategy and our models of practice are adopted abroad. We champion staff wellbeing, encourage ideas for service improvement and actively support career progression through trust wide leadership and culture change programmes.

Job responsibilities

Youll carry a small but complex caseload while providing professional and clinical leadership to your multi-disciplinary teammentoring staff, coordinating inductions and training, and fostering a supportive, learning-focused culture.

You will drive the design and implementation of community pathways that emphasise recovery, skill-building and prevention, working collaboratively with the QPP senior leaders to translate strategic priorities into everyday practice.

Collaboration is at the heart of this role: youll partner with inpatient wards, Home Treatment Teams, primary care, social services and voluntary sector organisations to ensure seamless, person-centred transitions across the care pathway.

In addition, youll champion quality improvement through audit, data-driven reviews and innovation projects, all underpinned by a commitment to compassionate, inclusive care.
