Job summary

Wirral Community Health & Care NHS Foundation Trust is seeking proactive Clinical Support Workers to join our Community Intermediate Care Centre (CICC). You will provide person-centred care under supervision of Registered Nurses and Therapists to support recovery and discharge planning for adults with sub-acute needs.

The unit promotes reablement and independence, with a 7-day rota and access to training and benefits as part of a supportive, multidisciplinary team.

Qualifications

• NVQ Level 2 in Health and Social Care, or demonstrable equivalent experience.
• Caring and compassionate attitude
• A strong desire to help people
• Good communication skills

Responsibilities

• Provide person-centred care to patients in line with their care plan.
• Care will be planned and assessed by Registered Nurses and Therapists.
• Work under the direct supervision of registered nurses or other members of the multidisciplinary team.
