Job description

The opportunity has arisen for a motivated, enthusiastic, and child-focused person to join the Children’s Community Nursing Team as a part-time Health Care Assistant. Candidates must have excellent communication skills and demonstrate a compassionate and empathetic approach to supporting children and families.

The successful applicant will assist qualified nursing staff within the Children’s Community Nursing Team to provide care and support to infants, children, and young people up to 18 years old in their own homes or other community settings. You will work autonomously in the community following training, within the level of competence for the post, to deliver agreed plans of care. There will be opportunities to progress professionally supported by training and learning opportunities.

Please indicate in your application whether you have a clean driving license. Previous experience working with children with special needs or healthcare is an advantage.

The post holder:

  1. Will perform agreed plans of patient care to identified groups of children/young people without direct supervision within the level of competency for the post.
  2. Act as a resource for families and children with healthcare needs.
  3. Work collaboratively with the acute ward, GPs, HVs, and other health professionals to enhance seamless care and reduce admissions to inpatient wards.
  4. Monitor and ensure standards of care are maintained and policies/procedures and protocols are adhered to.
  5. Always maintain respect and confidentiality.
  6. Maintain clear and accurate documentation using an electronic system, raising issues of concern with a qualified nurse where appropriate.
  7. Encourage and maintain good relationships with the CCNT, Specialist Children’s Hospitals, Hospices, Medical Staff, Community Nurse Specialists, and AHPs to provide the best care.

We employ more than 6,500 staff who deliver compassionate care from our two main hospitals, Calderdale Royal Hospital and Huddersfield Royal Infirmary, as well as in community sites, health centres, and patients’ homes. We also have nearly 150 volunteers at CHFT.

Our services include urgent and emergency care; medical; surgical; maternity; gynaecology; critical care; children’s and young people’s services; end-of-life care; outpatient and diagnostic imaging services.

We provide community health services, including sexual health services in Calderdale from Calderdale Royal and local health centres such as Todmorden Health Centre and Broad Street Plaza.
