Job summary

Locala Community Partnerships CIC is seeking a Clinical Specialist Occupational Therapist (Band 6) to join the South Kirklees Orthopaedic and Frailty team on a 22.5‑hour‑per‑week permanent basis. You will work in patients' homes and community settings, delivering rehabilitation and supporting independence.

HCPC registration and a driving licence are essential. The role involves MDT collaboration, falls and frailty management, and opportunities for ongoing training with supervision and annual

• Experience with falls, frailty, vascular and orthopaedic conditions
• Good communication and teamwork skills
• Driver with access to a vehicle for business use

Responsibilities

• Work as Band 6 Occupational Therapist in the South Kirklees Orthopaedic and Frailty team
• Provide community-based rehabilitation in patients' homes and community settings
• Promote independence and maximise patients' function
• Collaborate with multidisciplinary colleagues and contribute to falls prevention, frailty and orthopaedic care

Responsibilities

An opportunity has arisen for a Band 6 Occupational Therapist to join our South Kirklees Orthopaedic and Frailty team.

Locala community rehabilitation service provides high quality care to patients in their own homes, community rehabilitation facilities and care homes. We are patient centred and holistic in our approach and work with patients to maximise their independence.

The role requires a motivated and dynamic person to work within an established service, and it is essential that you can work as part of a multidisciplinary team. Experience of treating patients with falls, frailty , vascular and orthopaedic conditions is essential. Good communication skills and team working are essential qualities.

There are opportunities for training within the team.

This is a community-based role so being a driver with access to a vehicle for business use is an essential requirement for the role.
