Job description
Overview

This post is to support young adults with diabetes to self-manage their long-term condition. Transitioning from pediatric services into adult services. To support young people with insulin pump therapy and feel able to advocate on their behalf.

To communicate with people in a form and manner that is consistent with their level of understanding, culture, background and preferred ways of communicating.

Responsibilities
  • Have an existing knowledge of diabetes and insulin pump therapy. To achieve the best health and well-being outcomes for each young adult with diabetes referred or seen by the young adult diabetes service.
  • To facilitate care and ensure safe management of this group of patients in our care which includes managing those on insulin pump therapy.
  • To work with parents and or carers to meet the needs of the young adults to enable and empower them to self-manage and transition into adult services.
  • Support and advocate for young people with diabetes.
  • To provide specialist nursing support, advice and interventions within the multiagency ethos of young adults and family health services.
  • To provide case management of an identified caseload of young adults with highly complex diabetes by utilising and promoting evidence based care.
  • To support symptom management where necessary.
  • Be able to interpret continuous glucose monitoring data.
  • Have a problem-solving mindset and the ability to implement innovative approaches to patient care.
  • Act as the clinical expert and professional resource for patients and their carers and the multi-professional team.
  • Improve the overall standard and quality of diabetes care through targeted provision of evidence based practice and advice and support to healthcare providers in order to improve outcomes for young adults living with diabetes and contribute towards ending health inequalities.
  • Responsible for leading and working with colleagues in the direction of the specialist nursing services to both support and complement the organisation's business plans.
  • Directly or indirectly influence all aspects of care and management of patients within the service/speciality.
  • Provide direction and clinical leadership, working in partnership with the specialist team and multi professional team to enhance standards and best practice of care across the service.
  • Work collaboratively with primary care, acute trusts and other commissioned services to ensure seamless provision of care and support young adults with diabetes and their carers.
  • To work as an independent non-medical prescriber (adhering to Trust policy) with the ability to initiate and adjust medication within your field of expertise. Be able to initiate and support people on insulin pump therapy.

We provide advanced knowledge and skills in diabetes for young adults with diabetes and their carers across Leicestershire and Rutland. We aim to be an expert resource to support and educate people with diabetes, their families and other Health Care Professionals across organisational boundaries. We manage the introduction of diabetes treatment guidelines, protocols and complete audit and research projects. Support young adults with type 1 diabetes and insulin pump therapy. Be able to interpret CGM data. Have a problem-solving mindset and the ability to implement innovative approaches to patient care. Act as the clinical expert and professional resource for patients and their carers and the multi-professional team. Improve the overall standard and quality of diabetes care through targeted provision of evidence based practice and advice and support to healthcare providers in order to improve outcomes for young adults living with diabetes and contribute towards ending health inequalities.

  • Disclosure and Barring Service Check: If the post involves regulated activity, a DBS check is required. The Trust pays for the check initially and the cost is recovered from salary over a three-month period. Current DBS costs: £38 enhanced, £18 standard.
