Job summary

We have an opportunity for an Occupational Therapist, Physiotherapist or Speech and Language Therapist to join our Acquired Brain Injury (ABI) case management service.

LPT has been running the service since January 2024 and provides services to patients with ABI or strokes who require slow stream rehabilitation, overseeing their rehabilitation and ensuring they get the best outcomes possible. This is a rewarding job, working closely with the patient, their families and carers, and the specialist units providing their care and rehabilitation. We have close links with the specialist wards in the acute sector who refer in to the service, and work closely with them to facilitate timely discharges from hospital, whilst also ensuring the complex needs of the individuals are met. We also work with the Integrated Care Board, continuing health care and adult social care.

We are looking for someone passionate about working with ABI and stroke who is committed to improving care and outcomes for patients. You would be responsible for day-to-day team leadership, supporting and supervising the B6 case manager. Excellent team working, organisation & communication skills are essential for this role. Previous case management experience is desirable but not essential as you will be supported by the service leads to develop these skills.

This is a full-time role however we would be happy to consider part time hours for the right candidate.

Main duties of the job
  1. The post holder will have specialist knowledge and experience, and act as the expert for the multi-agency team and for patients within the relevant field.
  2. To work with acute health services and slow stream rehabilitation placement providers to facilitate/coordinate personalised care and rehabilitation for patients whilst on the ABI slow stream pathway.
  3. To work closely with the CINSS/ABI Clinical Service Leads to organise, develop, and maintain an efficient and effective ABI funded pathway service to LLR patients in the post-acute community setting.
  4. To actively participate and lead in the development of quality improvements for service delivery of the ABI pathway.
  5. To have responsibility for monitoring the implementation of ABI clinical guidelines and personalisation of rehabilitation programs within provider services.
  6. To work as a specialist clinician to utilise, develop and enhance Occupational Therapy/Physiotherapy/Speech and Language Therapy clinical and professional skills within the Service to deliver high standards of practice.
  7. The post holder will be responsible for holding their own caseload and supporting other members of the team with caseload management responsibilities.
  8. The post holder will be responsible for line management of the team.
  9. To deputise for the Clinical/Service Lead as appropriate.
  10. To work flexibly within the team, which may include supporting CINSS clinical pathways dependent on service needs.
