Zusammenfassung

Hinckley and Bosworth Medical Alliance Ltd is seeking a Clinical Pharmacist to support structured medication reviews and long-term condition management in GP practices across the network.

The role includes reconciling medicines after discharge, managing repeat prescriptions and providing medicines information, with CPD funded and multidisciplinary support; this is a fixed-term maternity cover with flexible hours and practice-based work.

Qualifikationen

• Masters degree in Pharmacy and GPhC registration.
• CPPE pathway completed or in progress (around 18 months).
• Prescribing qualification held, in progress, or planned after CPPE.

Aufgaben

• Carry out structured medication reviews and manage long-term conditions.
• Reconcile medicines after discharge and review medicines for new patients.
• Manage repeat prescription authorisation and issue acute prescriptions when appropriate.
• Support QOF and locally commissioned enhanced services; liaise with GP practices.
