Job summary

Hillton Primary Care Network is seeking a clinical pharmacist to join three GP practices on a 12-month maternity cover. The role focuses on structured medication reviews, long-term condition management, and medicines safety, performed autonomously within a supportive multidisciplinary team.

You will work full-time from January 2027, with local training provided. The ideal candidate will hold a pharmacy degree and GPhC registration; independent prescriber status is preferred.

Responsibilities

• Conduct clinical medicines management in GP settings.
• Provide face-to-face and remote structured medication reviews.
• Manage long-term conditions, including medicines optimisation and monitoring.
• Handle medicines on transfer of care and ensure safe prescribing.
• Process prescription requests, authorisations and reauthorisations.
• Promote medicines safety and contribute to quality improvement.
• Address public health and social care needs within GP practices.
• Collaborate within the PCN multidisciplinary team.
• Support to improve quality of care and operational efficiency.
• Work autonomously within clinical competencies.

This clinical pharmacist post sits within Hillton Primary Care Network, working across its three member practices on a 12-month maternity cover contract.
