Job summary

G DOC Ltd in Gloucestershire is seeking a Clinical Pharmacist to join the Dean Medical Practice network across Coleford, Brunston Surgery and Lydbrook Health Centre. You will lead medicines optimisation, support multidisciplinary teams, and contribute to patient-centred care within General Practice.

The role requires a pharmacist with experience in primary care, strong clinical judgement, and a commitment to improving patient outcomes across multiple sites in the Forest of Dean area.

Qualifications

• Undergraduate degree in pharmacy or MPharm.
• Current registration with General Pharmaceutical Council (GPhC).
• Completed or prepared to undertake PCPEP.
• Independent prescriber or working towards it is desirable.

Responsibilities

• Improve patients health outcomes and practice efficiency through medicines expertise.
• Primary contact within practices for medicines management queries.
• Lead medicines optimisation and conduct audits to improve care.
• Review and reconcile medicines on discharge and for new registrations.
• Develop and implement safe prescribing and review processes.
