Job summary

Qualifications
- Registered pharmacist with UK GPhC.
- Experience in community or general practice pharmacy.
- Independent prescribing qualification may be held or pursued.
Responsibilities
- Conduct structured medication reviews and optimise medicines.
- Provide clinical medicines management and prescribing advice.
- Support care homes to improve safety of medicines ordering and administration.
- Deliver patient-facing clinics and telephone advice on medicines.
- Collaborate with GP and AHPs to support safe prescribing and transfers of care.
