Job summary

Ellison View Surgery in Hebburn is seeking a motivated Pharmacist to join our friendly general practice team for 15 hours per week. You will work closely with GPs, nurses, and the wider clinical team to support safe and effective medicines management and high-quality patient care.

The role focuses on medication reviews, independent prescribing, and leading medicines optimisation initiatives while collaborating with the broader healthcare team to improve outcomes and patient safety.

Qualifications

• Must be a GPhC registered pharmacist with current professional registration.
• Include prescribing qualification (independent prescribing) or be enrolled in an approved pathway.
• Experience in primary care pharmacy and medicines optimisation is advantageous.

Responsibilities

• Conduct structured medication reviews and reconcile clinical letters and discharge summaries.
• Independently prescribe acute and repeat medications and provide patient consultations.
• Lead medicines optimisation and antimicrobial stewardship initiatives.
