Job summary

South Tyneside Health Collaboration (STHC) is seeking a Clinical Pharmacist to join our GP federation. You will deliver expert medicines optimisation and structured medication reviews within a multidisciplinary team, supporting safe, effective and patient-centred care across our practices.

You will work closely with GPs, nurses, technicians and other healthcare professionals to improve prescribing quality and patient outcomes, while ensuring compliance with regulatory standards.

Qualifications

• Registered Pharmacist with GPhC registration.
• Independent Prescriber qualification (or progressing).
• Commitment to CPD and ongoing professional development.

Responsibilities

• Deliver high-quality patient-facing consultations for medication reviews and medicines optimisation.
• Undertake Structured Medication Reviews (SMRs) for complex polypharmacy.
• Optimise medications according to national/local guidance and NICE recommendations.
• Promote medicines safety, governance and quality improvement within the practice.
• Provide expert prescribing advice to clinicians and staff and support MDT working.
