A permanent Clinical Pharmacist role within the pharmacy team of North Sedgemoor PCN, a network of five GP practices, with a base at the Clinical Hub. Qualifications
- MPharm degree or equivalent.
- Independent Prescriber qualification.
- GPhC registration.
- Evidence of clinical medication review and long-term condition management.
- Experience in multi-disciplinary teams.
Responsibilities
- Deliver structured medication reviews for complex polypharmacy.
- Provide clinical medicines advice to GPs and staff.
- Lead on medicines optimisation and prescribing safety.
- Manage prescribing for QOF and Enhanced Services.
- Support care home residents and related reviews.
- Reconcile medicines after hospital discharge.
- Train and mentor junior pharmacists and students.
