Job summary

Oxford University Hospitals NHS Foundation Trust is seeking a Clinical Pharmacist to support medicines management within the practice setting. You will identify areas for improvement, initiate and manage change, and contribute to cost‑effective prescribing.

You will provide medication review services via clinics, domiciliary visits and in residential/nursing homes, and lead long‑term condition management through prescribing best practice. Strong collaboration with partners is essential.

Qualifications

• Medicine management experience within a practice setting.
• Leadership of prescribing teams for long‑term condition management.
• Strong focus on improving quality of patient care and cost‑effective prescribing.

Responsibilities

• Develop and manage a medicines management plan and deliver patient services according to practice policy and guidance.
• Lead prescribing teams in best prescribing practice for long‑term condition management to improve quality of care and cost efficiency.
• Provide medication review services via clinics, domiciliary visits and in residential/nursing homes.
• Update patient medication records on clinical systems and ensure accurate documentation of advice and actions taken.
