Job summary

Mid Yorkshire Teaching NHS Trust is seeking a pharmacist with clinical experience to support the implementation of a new EPMA system. You will deliver a clinical service across several specialties, including medicines reconciliation on admission, prescription review, and discharge planning.

The role involves on-call duties, weekend rotas, and opportunities to develop leadership by supporting trainee pharmacists and collaborating with Advanced Clinical Pharmacists and the Digital Medicine team.

Qualifications

Master’s degree in pharmacy and GPhC registration are required.
Diploma in Clinical Pharmacy or equivalent advanced practice is expected.
Broad clinical experience across multiple specialty areas.

Responsibilities

Provide a clinical service in the relevant area.
Carry out Medicines Reconciliation on admission, prescription review and optimisation.
Plan and process prescriptions for discharge.
Contribute to on-call, late night, and weekend rotas.
Mentor foundation level and trainee pharmacists; collaborate with Advanced Clinical Pharmacists.
