Job summary

Circle Health Bath Clinic is recruiting a Clinical Pharmacist on a bank basis to cover busy periods and staff leave. You will conduct inpatient and outpatient medication reviews, advise patients on treatment plans, and support safe prescribing while ensuring availability and cost-effective use of medicines.

The role requires GPhC registration, hospital pharmacy experience, and strong therapeutics knowledge, with autonomous working alongside a multidisciplinary team.

Qualifications

• Degree in Pharmacy (MPharm, BSc or BPharm).
• GPhC registration as a Pharmacist.
• Previous experience in a clinical hospital pharmacy role.
• Strong knowledge of therapeutics and pharmaceutical legislation.

Responsibilities

• Conduct medication reviews for inpatients and outpatients, identifying interactions and optimisation opportunities.
• Provide tailored patient advice on treatment plans, side effects and lifestyle considerations.
• Support safe prescribing through reconciliation, screening and dose adjustment.
• Review and authorise prescriptions in line with guidelines and formulary restrictions.
• Promote safe, cost-effective medicine use and adherence to local/national policies.
• Collaborate with procurement, stock control and pharmacy technicians to ensure supply continuity.
• Monitor and report medication incidents and adverse drug reactions per MHRA guidance.

We have an opportunity for a Clinical Pharmacist to join their team of staff in the Pharmacy Department. This is a bank role working on an \"as and when required\" basis, you will be required to supplement staffing levels during busy periods and provide cover for existing staff in times of annual leave or absence. Duties of this role include:

• Conduct medication reviews for inpatients and outpatients, identifying interactions, contraindications, and opportunities for medicines optimisation.


• Provide tailored advice to patients on their treatment plans, including side effects, compliance, and lifestyle considerations.


• Support safe prescribing through reconciliation, clinical screening, and dose adjustment


• Review and authorise prescriptions in line with clinical guidelines, formulary restrictions, and regulatory requirements.


• Promote safe and cost-effective use of medicines by supporting adherence to local and national policies.


• Collaborate with procurement, stock control, and pharmacy technicians to ensure medicine availability and supply continuity.


• Monitor and report medication incidents, near misses, and adverse drug reactions in line with hospital protocols and MHRA guidance.
