Circle Health Group is seeking a Clinical Pharmacist to join the Bath Clinic pharmacy team on a bank, as-needed basis. The role offers an hourly rate of up to £35.00, reflecting experience and qualifications.

Qualifications

• This role requires a Pharmacy degree (MPharm/BSc/BPharm).
• GPhC registration as a Pharmacist is required.
• Experience in a clinical hospital pharmacy setting is preferred.
• Strong knowledge of therapeutics, clinical guidelines and pharmaceutical legislation.
• Excellent communication and clinical decision-making skills.
• Ability to work autonomously and in a multidisciplinary team.
• Proficiency with electronic prescribing and medication administration systems (EPMA).

Responsabilités

• Provide tailored patient advice on treatment plans, including side effects and adherence.
• Support safe prescribing through reconciliation, screening and dose adjustment.
• Review and authorise prescriptions per guidelines and formulary.
• Promote cost-effective medicine use in line with policies.
• Collaborate with procurement and pharmacy staff to ensure supply continuity.
• Monitor and report medication incidents per MHRA guidance.

We have an opportunity for a Clinical Pharmacist to join their team of staff in thePharmacy Department.

This is a bank role working on an “as and when required” basis, you will be required to supplement staffing levels during busy periods and provide cover for existing staff in times of annual leave or absence.

Duties of this role include:

• Conduct medication reviews for inpatients and outpatients, identifying interactions, contraindications, and opportunities for medicines optimisation.
• Provide tailored advice to patients on their treatment plans, including side effects, compliance, and lifestyle considerations.
• Support safe prescribing through reconciliation, clinical screening, and dose adjustment
• Review and authorise prescriptions in line with clinical guidelines, formulary restrictions, and regulatory requirements.
• Promote safe and cost-effective use of medicines by supporting adherence to local and national policies.
• Collaborate with procurement, stock control, and pharmacy technicians to ensure medicine availability and supply continuity.
• Monitor and report medication incidents, near misses, and adverse drug reactions in line with hospital protocols and MHRA guidance.

Applicants should meet the following criteria:

• A degree in Pharmacy MPharm, BSc or BPharm.
• GPhC registration as a Pharmacist.
• Previous experience in a clinical hospital pharmacy role.
• Strong knowledge of therapeutics, clinical guidelines, and pharmaceutical legislation.
• Excellent communication and clinical decision-making skills.
• Ability to work autonomously and as part of a multidisciplinary team.
• Confident using electronic prescribing and medication administration systems (e.g., EPMA).
