Main duties of the job
The role involves being a clinical pharmacist at our practice with a patient list size of around 18,000 patients. As a member of the general practice clinical pharmacy team you will be expected to deliver a high quality clinical pharmacy service to patients ensuring safe, clinically effective and cost efficient use of drugs as well as using expertise in independent prescribing to support better access to medicine.
Your role will be focused on general practice support, assessment of patients with long term conditions and ability to independently prescribe, medicines optimisation and reviews, aligning repeat prescription, acting on hospital letters and discharge letters, meeting local/national prescribing targets, working to the practice requirement and dealing with minor illnesses.We expect our clinical pharmacy team to evaluate the delivery, implementation and risks involved with our services as well as contribute to clinical governance agendas and regularly review policies and procedures with ideas to develop these in a multidisciplinary manner.
Your main duties include but are not limited to:- Medication reviews and queries including long term conditions- Audits for safe prescribing- high risk drugs monitoring- Practice CQC and MHRA Guidance compliance- Reconciling of practice purchased/administered drug- Completion/action of Docman/ tasks sent to pharmacist- MMT requirements and targets- Supervising pharmacy team admin staff and pharmacy technicians (if employed)

Clinical team is made up of eight GPS, two Pharmacists, two Nurse Practitioners, two Physician associates, two nurses, one HCA, two paramedics and attached community staff.
The surgery is open on Monday 8am-6.30pm, Tuesday 8am-7.30pm and Wednesday to Friday 7am-6.30pm

Salary: From £45,000-55,000 annual salary depending on experience and capability

- Working knowledge of EMIS/ Docman 10 systems
- Evidence of practical application of in depth therapeutic and clinical knowledge
- Ability to analyse, interpret and evaluate clinical evidence, data and information from various sources to make judgements, decisions and provide recommendations
- Logical and focused approach to problem solving
- Awareness of legislation govern-ing equality and diversity
- Understanding of confidentiality and data protection act
- Experience of QOF and clinical audit
- To enrolled in, or qualified from, an approved 18-month training pathway or equivalent that equips the Clinical Pharmacist to:
- a. be able to practice and prescribe safely and effectively in a primary care setting (for example, the CPPE Clinical Pharmacist training pathways and
- b. deliver the key responsibilities

- - Registration with the General Pharmaceutical Council (GPhc)
- - We prefer to have CPPE trained pharmacist in primary care but will consider any good candidate or newly qualified candidate willing to enrol in CPPE Clinical Pharmacist training in primary care.
- - Evidence of commitment to and able to demonstrate CPD
- - Appropriate defence indemnity (MPS/MDU)
- - Eligibility to practice in the UK independently- Disclosure Barring Service (DBS) check
Desirable
- Postgraduate diploma (or working towards) or equivalent evidence of clinical knowledge, competence and achievements in practice
- Independent Pharmacist Prescriber accreditation - or willing to work towards
- 2 years minimum experience working in GP
- Proven post graduate experience as a clinical pharmacist
- Membership with the Royal Pharmaceutical Society of Great Britain
