A leading healthcare provider is seeking a part-time pharmacist to support patient care and prescribe medications. The role involves working collaboratively with healthcare teams, conducting medication reviews, and ensuring compliance with health regulations. The ideal candidate will be an experienced GPhC registered pharmacist with strong communication and problem-solving skills, engaged in continuous professional development.
We are advertising a part time position during our opening hours 8am - 6.30pm Monday - Friday and alternate Saturdays 9am - 4.30pm.
Thepost holder will work within their clinical competencies to provide specialistpharmaceutical support to the practice including working withmembers of the wider healthcare team to improve care and support safe andeffective prescribing.
Thepost holder will need to be proactively involved in quality improvement andaudit as well as managing some aspects of the Quality and Outcomes Frameworkand Impact and Investment Fund requirements and Local Enhanced Services.
Toreconcile medicines following discharge from hospitals or intermediate care andworking with patients and community pharmacists to ensure patients receive themedicines they need post-discharge and to reduce risk of readmission.
Tomanage repeat prescription requests, resolving queries where possible withinscope of practice. To ensure patients have appropriate monitoring tests inplace when required.
Conduct medication reviews.
Participatein review and setting of policies relating to medicines management andprescribing and help ensure practices prescribe in accordance with localguidelines and formulary
Tomonitor network practices prescribing against the local health economy andmake recommendations for GPs or amend within scope of practice, based on findings.
Tomaintain own clinical and professional competence and be responsible for owncontinuous professional development
Ensure full compliance with Care Quality Commission standardsfor safe and effective care
Taking responsibility for owndevelopment with relevant evidence-based knowledge and competence in allaspects of the role to meet clinical governance guidelines for ContinuingProfessional Development (CPD) and a Personal Development Plan (PDP).
Commit to take and pass thenecessary Physician Associate re-certification exams (currently every 6 years)
We are an established, well motivated team with 2 GP Partners, salaried and locum GPS,1 Nurse Practitioner, 2 practice nurses, 2 Health Care Assistants and an excellent admin team. Our list size is just under 7,000 patients. We have a number of ARRS roles and a part time in house pharmacist. Our last CQC rating was Good.

As an employee of the PCN you will need to be aware of your responsibility in relation to safeguarding and protecting children and vulnerable adults. You will need to be aware of our procedures and know how to contact named professionals, within the safeguarding team for advice and support.
