We are looking to welcome an experienced Clinical Pharmacistto join our dynamic and supportive team at Humberstone Medical Centre. Thisvacancy is up to 15 hours per week, salary depending on experience.
Were looking for enthusiastic and forward-thinkingpharmacist to complement the team. Thesuccessful candidate will hold clinics for patients requiring medicationreviews, manage prescription queries and be involved in long term conditionmanagement. We welcome applicants with or without GP experience. If you havethe right skills and qualities, we can provide on-going support to developprofessionally.

We are a large and friendly team including 5 GP Partners, 3Salaried Drs, 2 Pharmacists, 1 Pharmacy technician, 3 Nurses, 2 HCAs, an FCP, aSocial Prescriber, care coordinators and administrators.
We are a teaching practice and take GP Registrars andMedical students on rotation.
We are part of Salutem Primary Care Network and take a leadrole in it with the previous and current Clinical Director both being partnersin the practice. Salutem PCN has an excellent reputation for collaborativeworking in multiple projects and seeking to maintain high standards of patientcare in all 5 practices within it.

Provide telephone support for patient queriesrelating to medication.
Undertake structured medication reviews withmulti morbidity and polypharmacy and implement changes whilst ordering relevantmonitoring.
Collaborate with the wider healthcare team toimprove safety of medicines ordering and administration.
Provide support for the pharmacy technician formanagement of medicines following hospital discharge and clinic visits.
