Job summary

Exmoor Surgery in North Kensington, London, is seeking an enthusiastic Clinical Pharmacist to join our friendly multidisciplinary primary care team for 2–3 days per week, with an option to work from home on a Friday. The usual day is 9:00 a.m.

to 5:00 p.m., combining telephone consultations, clinical reviews and protected administrative time. The successful candidate will support safe, effective and cost-efficient medicines management, working closely with GPs, nurses and the wider practice

Qualifications

• GPhC registration is required.
• Experience of working in a GP practice is required.
• GCSEs and A-levels are expected.

Responsibilities

• Practise as an Independent Prescriber within your scope of practice.
• Conduct face to face and telephone consultations with patients, including supporting patients with minor illnesses and self-limiting conditions.
• Conduct structured medication reviews and repeat medication reviews for patients within the practice.
• Undertake planned care medication reviews, including cardiovascular, renal and metabolic and diabetes reviews.
• Manage acute medication requests and prescribing queries.
• Review hospital discharge letters and clinic correspondence, ensuring appropriate medication changes are implemented.
• Reconcile medicines following patient discharge from hospital or attendance at outpatient clinics.
• Review medications for newly registered patients.
• Encourage safe, effective and cost efficient prescribing within the practice.
• Review medication safety alerts and ensure appropriate actions are identified and completed.
• Provide subject matter expertise on medication monitoring and support the implementation and maintenance of robust monitoring systems.
• Carry out targeted medication reviews for patients taking high risk medicines, including DMARDs.
• Reauthorise repeat medication within your scope of practice.
• Identify patients suitable for repeat prescribing and liaise with relevant clinicians where required.
• Support the development and implementation of safe and effective repeat prescribing systems.
• Review and implement safe prescribing systems for vulnerable patients and support improved medication compliance.
• Provide advice and clinical support to GPs, nurses and the wider multidisciplinary team regarding medicines management and prescribing.
• Support clinicians with the management of patients with drug and alcohol dependencies.
• Actively signpost patients to the appropriate healthcare professional or service where required.
• Support the practice in achieving relevant QOF targets and other primary care objectives.
• Work collaboratively with the two existing Clinical Pharmacists, GPs, nurses and wider practice team.
• Use SystmOne effectively to document consultations, medication reviews, prescribing decisions and clinical activity.
• Contribute to clinical governance, medicines optimisation and the ongoing development of pharmacy services within Exmoor Surgery.
• Administer influenza and travel vaccines where appropriately trained and authorised.

Job summary

Exmoor Surgery is looking for an enthusiastic and experienced Clinical Pharmacist to join our friendly and supportive multidisciplinary primary care team for 2 to 3 days per week. Ideally, the working days will be Thursday and Friday, although there is some flexibility, including the option to work from home on a Friday.

The Clinical Pharmacist will support the delivery of safe, effective and cost efficient medicines management within the practice, working closely with GPs, nurses, our existing Clinical Pharmacists and the wider practice team.

The usual working day is 9:00 a.m. to 5:00 p.m., with a balanced mixture of telephone consultations, clinical reviews and protected administrative time.

The successful candidate should be organised, clinically confident and comfortable working both independently and collaboratively. Previous experience in general practice or primary care would be an advantage, and an Independent Prescribing qualification is desirable.

Experience of working with SystmOne would be beneficial.

This is an excellent opportunity for a Clinical Pharmacist to become an integral part of a supportive multidisciplinary team and contribute to the delivery of high quality patient care and medicines optimisation within Exmoor Surgery.

Main duties of the job

Qualifications

Essential

• GPhC registration
• Experience of working in a GP practice
• GCSEs are considered alongside A-levels, with most schools of pharmacy expecting a minimum of five GCSEs including maths, English language and science
