Surrey Heath Community Providers Limited is seeking a Clinical Pharmacist to join the growing clinical pharmacy and multi-disciplinary PCN team. The role covers two GP practices and care homes in Surrey Heath, four days per week, with a focus on medicines management and safe, effective prescribing across registered populations.

This is a training role requiring CPPE/PCPEP training; successful candidates will implement medicines optimisation, support the PCN medicines workstreams and contribute

Qualifications

  • GCSEs including English and Maths (C and above) or equivalent.
  • Masters degree in pharmacy (MPharm) or equivalent.
  • Post-graduate clinical diploma level or equivalent training/experience.
  • Will ingness to undertake approved primary care pharmacy educational pathway (PCPEP).
  • Willingness to undertake independent prescribing training.

Responsibilities

This if a part time role for Medicine Management Optimisation across the disciplines of both General Practice and Care Homes. If you are interested in working in this dynamic environment then please come and join this enthusiastic dynamic team.

As a Clinical Pharmacist in Surrey Heath you will join the growing clinical pharmacy and multi-disciplinary PCN team. Working across a nominated general practice to support the responsibilities and deliverable of the Surrey Heath Primary Care Network.

This role is based across two practices and the Surrey Heath footprint working four days per week.

You will be responsible for supporting the practice(s) to implement effective medicines management, identifying areas for improvement, and initiating and managing change. In addition the role will be responsible for supporting the Primary Care Network to implement the medicines optimisation requirements of the new Network Contract, Direct Enhanced Service for enhanced health in care homes and other PCN medicines management responsibilities.

Main duties of the job

This is a training role which requires thesuccessful applicant to complete a CPPE course, therefore please ensure youunderstand the training requirements. These roles have been designed to beinteresting and fulfilling roles, providing a comprehensive range ofappropriate clinical pharmacy services to general practice in Surrey Heath. Thepost holders will undertake resident/patient-facing activities, and associatedmedicines management support activities, to ensure effective and efficient useof medicines within the practices and across their registered populations.

The post holder will be supported tohelp develop the role and work to improve the care and outcomes for thepatients across Surrey Heath PCN. To play a pivotal role in the delivery ofhigh quality primary health care within Surrey Heath

Person Specification
Qualifications
  • GCSEs including English and Maths (C and above) or equivalent
  • Masters degree in pharmacy (MPharm) or equivalent
  • Post-graduate clinical diploma level or equivalent training/experience.
  • Have completed, be enrolled in, be undertaking or prepared to undertake an approved primary care pharmacy educational pathway (PCPEP), e.g. the CPPE Primary care pharmacy education pathway or Medicines optimisation in care homes training pathway for pharmacy professionals.
  • Willingness to undertake independent prescribing training.
Experience
  • In depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare.
  • An appreciation of the nature of GP's and general practices.
  • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
  • Excellent interpersonal, influencing and negotiating skills.
  • Excellent written and verbal communication skills.
  • Experience of motivational interviewing and able to train other staff.
  • Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences.
  • Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions
  • Familiarity with GP systems.
  • Able to obtain, analyse and interpret complex technical information, and present reports.
  • Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GP's when appropriate.
  • Demonstrates accountability for delivering professional expertise and direct service provision.
  • Minimum of 5 years post-qualification experience.
Professional Registration
  • Registration with General Pharmaceutical Council.
  • A member of or working towards Faculty membership
  • of the Royal Pharmaceutical Society.
