Job summary

The Uplands Medical Practice is seeking an experienced Advanced Pharmacist Practitioner to join our multidisciplinary primary care team. You will provide expert pharmaceutical care, support chronic disease management, and contribute to safety, quality and cost-effective prescribing.

Job summary

The Uplands Medical Practice is seeking a motivated and experienced Advanced Pharmacist Practitioner to join our multidisciplinary primary care team. This is an exciting opportunity for a clinically focused pharmacist to play a key role in improving patient outcomes, enhancing medication safety, and supporting the delivery of high-quality care across the practice.

The successful candidate will work closely with GPs, nurses, and other healthcare professionals to provide expert pharmaceutical care, support chronic disease management, and contribute to regulatory compliance and quality improvement initiatives.

Main duties of the job

The Advanced Pharmacist Practitioner will provide comprehensive clinical pharmacy services within the practice, including conducting structured medication reviews, managing prescriptions and repeat prescribing processes, implementing medication safety alerts and MHRA guidance, and supporting safe, effective, and cost-efficient prescribing. The role will also involve supporting patients with long-term conditions such as diabetes, hypertension, asthma, and COPD through medication optimisation, monitoring, treatment reviews, and patient education. In addition, the post holder will review and manage blood test results related to medication monitoring, contact patients regarding follow-up actions where required, and ensure compliance with clinical safety standards. The successful candidate will assess and manage minor ailments within their scope of practice, providing treatment advice, prescribing where appropriate, and making referrals when necessary. They will also play an important role in supporting the practice's quality, safety, and governance agenda by contributing to CQC compliance, clinical audits, quality improvement projects, medicines management initiatives, and the development of policies and protocols to ensure the delivery of high-quality patient care.

Job responsibilities

Key Responsibilities

Medicines Optimisation and Prescribing

• Conduct face-to-face and remote structured medication reviews.
• Support safe, effective, and cost-efficient prescribing.
• Process and manage prescription requests and medication queries.
• Review and implement medication safety alerts, MHRA guidance, and prescribing updates.
• Undertake medicines reconciliation following hospital discharge and clinic correspondence.
• Manage repeat prescribing systems to ensure safe and appropriate medication use.

Chronic Disease Management

Provide clinical support and patient reviews for long-term conditions, including:

• Diabetes
• Hypertension
• Asthma
• COPD

This may include medication optimisation, treatment escalation, patient education, monitoring, and adherence support as required.

Blood Test and Medication Monitoring

• Review blood test results related to medication monitoring.
• Contact patients by telephone regarding abnormal or outstanding results.
• Arrange and monitor required follow-up investigations in line with clinical protocols.
• Ensure compliance with medication monitoring requirements and clinical safety standards.

Minor Ailments Service

• Assess, diagnose, and manage patients presenting with minor illnesses and common ailments within your scope of practice.
• Provide treatment advice, prescribing where appropriate, and onward referrals when necessary.

Quality, Safety and Governance

• Support the practice in achieving and maintaining CQC compliance and quality standards.
• Assist with clinical audits, quality improvement projects, and medicines management initiatives.
• Identify and mitigate prescribing risks to improve patient safety.
• Contribute to practice policies, protocols, and medicines-related governance activities.

Essential Requirements

• Registered Pharmacist with the General Pharmaceutical Council (GPhC).
• Independent Prescriber qualification (or working towards obtaining one).
• Experience in Primary Care, General Practice, PCN, or Community Pharmacy.
• Strong knowledge of medicines optimisation, prescribing, and long-term condition management.
• Excellent communication, consultation, and organisational skills.
• Ability to manage workload independently while working collaboratively within a multidisciplinary team.

Desirable Requirements

• Previous experience supporting CQC inspections and evidence gathering.
• Experience with EMIS Web and NHS clinical systems.
• Experience running clinics for chronic disease management.
• Advanced clinical assessment and consultation skills.

Qualifications

Essential

• Master of Pharmacy (MPharm) degree
• Registered Pharmacist with the General Pharmaceutical Council (GPhC)
• Independent Prescriber (IP) qualification
• Postgraduate Diploma or MSc in Clinical Pharmacy/Advanced Clinical Practice.

Desirable

• Advanced clinical assessment qualification such as training in chronic disease management.
