Job summary

An NHS hospice is seeking a Coordination Centre Clinical Nurse Specialist to manage clinical coordination of care for patients. The ideal candidate will have experience in palliative care and will be a key part of a dedicated team enhancing service delivery and patient access.

Qualifications

  • Essential: Registered Nurse and degree or modules in Palliative Care.
  • Proven experience in palliative care and managing teams.

Responsibilities

  • Provide clinical oversight for referrals and ensure timely access to services.

  • Salary: Rennie Grove Peace Band 7 - £46,148 - £52,809 per annum (pro rata)

Main duties of the job

The purpose of this role is to be part of a small team of senior nurses who are responsible for the clinical coordination of care for patients and families referred to Rennie Grove Peace.

This role will suit someone who is:

  • An experienced palliative care nurse who would like to be part of a new development which aims to ensure equity of access to Rennie Grove Peace Services.

  • Being the point of contact for patients, families, clients and professionals who contact the hospice, dealing with all clinical enquiries.
  • Providing clinical oversight and triage for referrals to ensure that the patients and family members get the right Rennie Grove Peace service in a timely manner.

  • Proven experience of effective team working.
  • IT literate including a good knowledge of Excel Word, a clinical IT system.
  • Excellent written and verbal communication skills.
  • Good organisational skills.
  • Teaching and presentation skills.
  • Advanced assessment skills and specialist symptom control knowledge to plan, implement and evaluate nursing care.
  • Drive and enthusiasm with a positive approach.
  • Caring and empathetic approach
  • Flexible approach and attitude to working.
  • Commitment to Rennie Grove Peace hospice car emission and aims.
  • Sensitivity and understanding to the needs of your team and customers, many of whom may have experienced Rennie Grove Peace Hospice care services or be bereaved.
