Main duties of the job

In this role, youll provide expert symptom management and palliative care to children across all settings whether theyre at the hospice, in hospital, or out in the community. Youll act as the named liaison for children who are hospitalised and already known to us, making sure their care is well-coordinated and that both they and their families feel consistently supported. Youll also work closely with hospital-based paediatric and palliative care teams to identify new children who might benefit from a referral to the hospice, helping us reach families who need us early on. As part of a truly collaborative approach, youll take part in multidisciplinary meetings, discharge planning, and advance care planning, ensuring care remains joined-up and centred around the child. Alongside this, youll be part of the hospices senior nursing team, offering education, support, and leadership to help shape the way we deliver care.

This role is based at our beautiful woodland hospice in Woodford Green, offering a peaceful, child friendly environment, free onsite parking and easy access from the Central line.

As a key member of the clinical leadership team, the Clinical Nurse Specialist (CNS) with a Hospital Liaison focus will ensure high-quality, family-centred care for babies, children and young people with life-limiting or complex needs. This post is pivotal in strengthening relationships between Haven House, tertiary hospitals, and community providers to ensure seamless transitions of care, safe discharges, and coordinated support during hospital admissions.

The post-holder will deliver advanced clinical care and provide expert support for symptom management, palliative and end-of-life care, and emotional support for families navigating complex hospital journeys. This role requires strong interagency collaboration, expert clinical decision-making, and a proactive approach to supporting hospital-to-hospice transitions.

In addition to clinical leadership, the post-holder will contribute to education, service development, risk management and the supervision of a team of registered and unregistered staff.

Key Responsibilities

Clinical

• Act as the key link between hospice and hospital services, especially PICU/NICU, to facilitate timely discharge planning and coordinated care.
• Support families emotionally and practically during hospital stays and assist in navigating discharge and care planning.
• Develop, implement and review advance care plans and symptom management plans in collaboration with hospital and community professionals.
• Work flexibly across all settings (hospice, hospital, home) in response to the needs and preferences of children and families.
