Job Description: Crisis Resolution and Home Treatment Team for Older Adults

The Crisis Resolution And Home Treatment Team For Older Adults is a well-established, friendly, and supportive multidisciplinary team based in Fulbourn, Cambridge/Peterborough, and Fenland. The team offers intensive support and treatment at home and in care homes for older adults with mental health challenges, including the behavioral and psychological symptoms of dementia.

The services provided include:

  1. Crisis intervention, intensive treatment, support, risk assessment, and management to prevent unnecessary hospital admissions and facilitate earlier discharge from mental health hospitals.
  2. Dementia Intensive Support, offering practical and psycho-educational care within patients' homes and care homes.

We are a strong, passionate, and cohesive team with extensive clinical experience and knowledge. We are seeking a team member who possesses the following attributes:

  • Experience in conducting clinical assessments, including risk assessments.

The successful candidate will work within the Crisis Resolution and Home Treatment Team for older people, providing mental health services seven days a week in a community setting. Responsibilities include:

  • Conducting psychosocial assessments for individuals presenting with various mental health issues.
  • Working in partnership with service users and carers to provide treatment in the least restrictive environment.
  • Taking on the role of care coordinator, ensuring comprehensive assessment, care planning, and review.
  • Supporting and guiding Band 6 team members in care planning and treatment pathways.
  • Assessing individuals at risk of hospital admission, providing community-based care, or facilitating hospital admission and discharge.
  • Providing clinical risk and mental health assessments, formulation, and short-term contingency planning during crises.
  • Acting as a role model and promoting effective teamwork to meet individual client needs.
  • Supporting the team lead, including supervision and training as needed.
  • Providing crisis and home treatment services, including dementia support.
