Job summary

Cheshire and Wirral Partnership NHS Foundation Trust is seeking a Clinical Lead in Community Nursing for the Chester area. This Band 6, permanent role provides clinical leadership across a designated community caseload and works with a broad MDT.

Qualifications

• First level registered general adult nurse with current registration.
• SPC qualification in District Nursing or equivalent with caseload management experience.
• Evidence of teaching and assessing.

Responsibilities

• Provide a highly visible clinical profile with leadership and oversight of a designated community caseload.
• Work collaboratively with wider health, social care and third sector partners to meet patient needs.
• Ensure patients receive the right care, by the right person at the right time and place.
• Place the patient at the centre, creating a streamlined journey with minimal delays and improved independence.
• Provide direction within the caseload considering health and social care agendas and local initiatives.
• Be aware of local public health profiles and their influence on care.
• Understand the skills required from the Community Care Team.
• Focus on delivery of person-centred care.
• Provide evidence-based care through caseload management and monitoring of standards.
• Provide clinical leadership.
