Job summary

Chester South Care Community Team is seeking a Clinical Lead to provide clinical leadership within a multidisciplinary team including Team Manager, Enhanced Clinical Practitioner, Clinical Leads, Community Nurses and other specialists.

The post is full time (37.5 hours per week) and involves safeguarding, collaboration with partners, and a patient-centered approach to care across the community setting.

Qualifications

• Enthusiastic, self-motivated and provides confident support and clinical leadership to clinicians in collaboration with other professionals.
• Excellent communication skills and ability to work with patients and their families/carers; safeguarding when necessary.

Responsibilities

• Provide a highly visible clinical profile with leadership and clinical oversight of a designated community caseload
• Collaborate with wider health, social care and third sector to meet patient needs
• Ensure patients receive the right care, by the right person, at the right time and place
• Place the patient at the centre of all decisions to streamline the patient journey and improve independence and care quality
• Provide direction within the caseload considering local health and social care agendas and initiatives
• Be aware of local public health profiles and how they influence required care
• Understand community profile skills for the Community Care Team
• Focus on delivering person-centred care
• Provide effective, evidence-based care through caseload management and monitoring of care standards
