Job summary

Dorothy House is offering an internal opportunity for a Community Palliative Care Team (CPCT) Clinical Lead in Winsley, Wiltshire. This 34-hour per week role runs over five days (Mon, Tue, Thu, Fri) with a fixed-term 6-month contract and a community-based location.

You will provide professional leadership, coaching, operational oversight, and maintain a limited clinical caseload while guiding workforce development and strategic service improvements.

Qualifications

  • Registered Nurse with current NMC registration.
  • Postgraduate qualification in palliative care or equivalent leadership experience.
  • Significant clinical experience in specialist palliative and end of life care.
  • Experience leading or supervising clinical teams and staff development.
  • Ability to assess, manage and coordinate complex patient care in community settings.
  • Excellent communication, influencing and partnership-working skills.
  • Experience with service improvement, quality improvement or change initiatives.
  • Full UK driving licence with access to a vehicle.

Responsibilities

  • Provide clinical and operational leadership to Community Palliative Care Teams.
  • Coach and develop Band 5 and 6 colleagues; foster continuous learning.
  • Coordinate day-to-day caseloads, resources and workforce deployment.
  • Maintain a limited specialist palliative care caseload and undertake complex care planning.
  • Build partnerships with health and social care colleagues and care homes.
  • Lead quality improvements, service development projects and clinical audits.
  • Support recruitment, induction and performance development within the team.
  • Represent the service in meetings and influence future care directions.

COMMUNITY PALLIATIVE CARE TEAM - CLINICAL LEAD | Band 7 | £25.26 - £28.90 per hour

Fixed Term Contract: 6 months
Part Time: 34 hours over 5 days a week (Monday, Tuesday, Thursday, Friday).

WHY THIS ROLE?

We are delighted to offer this exciting internal opportunity for an experienced clinician to take the next step in their leadership journey as a Community Palliative Care Team (CPCT) Clinical Lead.

This is a key leadership role within our Community Palliative Care Service, combining clinical expertise with team leadership, service development and quality improvement. Working closely with the Community Matron and wider Care Services leadership team, you'll help shape the future of community palliative care while supporting colleagues to deliver outstanding care to people living with life-limiting illness and those important to them.

As a Clinical Lead, you'll provide professional leadership, coaching and operational oversight to a multidisciplinary community team, championing innovation, collaboration and continuous improvement. You'll continue to maintain a limited clinical caseload, whilst playing a pivotal role in workforce development, partnership working and the delivery of our strategic ambitions across community services.

WHAT WILL THE ROLE INVOLVE?

  • Providing clinical and operational leadership to Community Palliative Care Teams, ensuring safe, effective and high-quality care delivery
  • Supporting, coaching and developing Band 5 and Band 6 colleagues, creating a culture of continuous learning and professional growth
  • Managing the day-to-day coordination of community caseloads, resources and workforce deployment
  • Maintaining a limited specialist palliative care caseload, undertaking complex assessments and care planning for patients and families
  • Building strong partnerships with health and social care colleagues, care homes and community organisations to improve patient outcomes
  • Leading quality improvement initiatives, service development projects, clinical audit and the implementation of new models of care
  • Supporting recruitment, induction, supervision, performance development and succession planning within the team
  • Representing the service at internal and external meetings, helping to influence the future direction of palliative and end of life care services

ABOUT YOU

Please Note that only existing Dorothy House employees and volunteers are eligible to apply for this role.

Essential requirements:

  • Registered Nurse with current NMC registration
  • Postgraduate qualification, or equivalent level of knowledge and experience, relevant to palliative care, leadership or advanced practice
  • Significant clinical experience in specialist palliative and end of life care
  • Experience of leading, managing or supervising clinical teams and supporting staff development
  • Demonstrable ability to assess, manage and coordinate complex patient care within a community setting
  • Excellent communication, influencing and partnership-working skills
  • Experience of service improvement, quality improvement, audit or implementing change within clinical services
  • Full UK driving licence with access to a vehicle

Desirable requirements:

  • Independent Prescriber qualification, or willingness to work towards this
  • Teaching, mentorship or assessor qualification
  • Community nursing qualification or substantial community palliative care experience
