About The Role

Hours: 37.5 hours per week

Location: Thorne

We are looking to hire a Clinical Lead responsible for managing daily complex support packages and ensuring high standards of person-centred care. The role also involves supporting referrals and transitions for new services under the Enhanced Supported Living Framework.

Key Responsibilities Include:

1. Lead initial referrals for complex cases, including forensic, mental health, personality disorder, and dual diagnosis.
2. Provide clinical leadership during transitions, ensuring appropriate support and transition plans are created and implemented.
3. Promote effective collaboration across services during transitions.
4. Lead, motivate, mentor, and coach the Transitions team for ongoing development.
5. Collaborate with the recruitment team to meet high-quality staffing needs aligned with transition plans.
6. Work with local RM to ensure seamless service transfer.
7. Partner with PBS colleagues to support and evidence PBS practices.
8. Contribute to the development of tailored service-specific training with specialist trainers.
9. Implement tools and techniques to support safe clinical practice.
10. Collaborate with clinical roles across the organization.
11. Provide ongoing support to complex cases post-transition.
12. Attend MDT and relevant professional meetings.
13. Partner with housing providers and colleagues to identify suitable housing options.
14. Demonstrate advanced clinical skills and advise on service development and care delivery in partnership with individuals, carers, colleagues, and external agencies.
