Job summary

The Garth Surgery is looking for a Clinical Coder / Care Navigator to join our friendly team.

This is a dual role split equally across each working day (we know that coding for a full day can be hard going plus we like to offer variation and job satisfaction).

Main duties of the job

Medical terminology and an existing understanding of read coding is essential for this role. You will identify information from documentation to be added to the clinical system, often linking in with other clinicians and the pharmacy/prescription team.

We have protocols that we follow and you will be trained to do so by our experienced coder.

The Garth Surgery is a supportive place to work, we have a friendly team and participate in practice team building and social events. We are a partnership of 6 GPs, 1 Salaried GP, 3 ANPs, 3 PNs, 2 Nursing Associates, 1 HCA, and 1 Phlebotomist, plus many other healthcare providers attending the surgery to provide care to patients via our Primary Care Network (East Cleveland PCN).

Job responsibilities

The following are the core responsibilities of the clinical coder:

1. Accurately read-coding data into the patient record
2. Identifying relevant clinical information documented in patient correspondence, coding appropriately and completing any detailed actions
3. Input and extract data as required to support practice reports
4. Action all incoming emails and mail in a timely manner
5. Scanning of patient related documentation and attaching scanned documents to patients healthcare records
6. Input data into the patients healthcare records as necessary
7. Answer incoming phone calls, transferring calls or dealing with the callers request appropriately
8. Carry out system searches as requested
9. Maintain a clean, tidy, effective working area at all times
10. Support all clinical staff with general administrative tasks as requested

The following are the core responsibilities of the Care Navigator:

1. Maintaining and monitoring the practice appointment system
2. Process personal, telephone and e-requests for appointments
3. Answer incoming phone calls, transferring calls or dealing with the callers request appropriately
4. Signpost patients to the correct service
5. Initiating contact with and responding to requests from patients, team members, and external agencies
6. Read code data on Systmone as per the practice workflow
7. Photocopy documentation as required
8. Provide administrative support for the GPs and Nurses
